As you know, Livonia Central School takes the privacy of your student’s information seriously. As part of our ongoing efforts to better secure student information, Livonia Central School has licensed a Health Services Portal to provide you with a convenient, 24-hour access online to items in your child’s health information from anywhere through the Internet. Using a secure username and password, you can view district-designated health information, communicate with health and wellness staff, review and sign forms and even provide the district with new information from outside health services received by your child.
This system will be used for Livonia staff and parents to complete a daily COVID attestation.
Note: If you prefer to have the convenience of the health portal on your mobile device you can download the App from the links at the bottom of the page. Please complete the registration steps first.
Step 1 - Have the following information available
Student last name
Student date of birth
Student school ID - Where can I find this?
Parent primary contact email - this is the email where you receive school communications and use for your SchoolTool login. If you aren't sure of what email you have on file please contact the LCS Help desk 346-4000 x1233 or firstname.lastname@example.org
Step 2 - Go to the Health Office Livonia Central School District Registration site.
Step 3 - You will receive an email with your user name and temporary password. If you haven’t received the email after 10-15 minutes, please check your spam folder in your email.
Step 4 - Once you login and change your temporary password your Health Portal account is active! You may need to provide an electronic signature that starts with /S/ (no quotes are needed around it).
Step 5 - If you have more than one student, please add all of them to your account.
Step 6 - Once you have completed the registration process you can login and complete the daily health attestation (How to guide to complete the daily health attestation):
